Job Summary
The Hospital Pharmacist is responsible for ensuring the safe, accurate, and appropriate use of medications throughout the hospital. The role provides pharmaceutical care, reviews medication orders, supports clinicians with medication-related information, monitors medication safety, and contributes to optimizing medication therapy and patient outcomes.
Key Responsibilities
- Review and accurately dispense medications in accordance with prescriptions, medication orders, and established pharmacy procedures.
- Verify medication orders for appropriate drug selection, dosage, route, frequency, interactions, allergies, and potential contraindications.
- Provide medication-related information and clinical support to physicians, nurses, and other healthcare professionals.
- Monitor patients’ medication therapy for effectiveness, safety, adverse reactions, and potential drug interactions.
- Identify and report medication errors, adverse drug reactions, and other medication-related concerns.
- Provide appropriate medication counseling and education to patients and caregivers.
- Support clinicians in optimizing medication therapy based on patient-specific needs.
- Ensure proper storage, handling, labeling, and distribution of medications.
- Maintain accurate pharmacy records and documentation in accordance with hospital policies and regulatory requirements.
- Monitor pharmacy inventory and ensure adequate availability of essential medications and supplies.
- Monitor medication expiry dates, storage conditions, and stock rotation.
- Participate in medication safety, quality improvement, and infection prevention initiatives.
- Support the implementation and adherence to pharmacy policies, procedures, and medication-use protocols.
- Maintain confidentiality and comply with applicable professional and hospital standards.
- Collaborate with multidisciplinary healthcare teams to promote safe and effective patient care.
Minimum Qualifications
- Bachelor’s Degree in Pharmacy (B.Pharm or equivalent).
- Current and valid professional pharmacist license/registration.
- Knowledge of clinical pharmacology, medication therapy, and pharmaceutical care.
- Knowledge of medication safety practices and applicable pharmacy standards.
Experience Required
- 3–5 years of professional experience in a hospital or clinical pharmacy setting.
- Experience with medication dispensing, medication order review, inventory management, and pharmaceutical care.
- Hospital pharmacy experience involving multidisciplinary clinical teams is preferred.
