Ascending aorta could be safely reached in every patient through the 4th ICS. Access to the ascending aorta is one of the cornerstones of TCRAT technique – Total Coronary Revascularization via left Anterior Thoracotomy.
Special maneuvers are routinely performed to make the proximal anasotomosis easy and safe: selection of place for anastomosis, conduit length measurements, heart displacement, aortic tape, position, clamp application. Special retractor with concave upper blade that doesn’t obstruct the view is also extremely important.
