Atretic coronary venous sinus ostium ( CS) , Total Anomalous Coronary Venous Drainage to LT BCV (TACVD).
The middle & great cardiac veins (MCV & GCV) drain into a left sided vertical vein ( same course of persistent LSVC) to left BCV & also a vein draining the anterior wall of the RV ending into the left vertical vein . U must notify the surgeon this item so as not to ligate this thought persistent LSVC / vertical vein to any reason as in partial cavo-pulmonary connection (Glenn shunt) otherwise catastrophic sequale will happen.
so in any congenital CT study make sure the CS ostium is patent & not atretic.
