We illustrate the integration of EIT-derived ventilation mapping to guide personalized positioning strategies in a non-intubated patient with moderate ARDS, demonstrating its potential to optimize alveolar recruitment while mitigating positional intolerance.<\/p>\n<h3 id=\"ember64\" class=\"ember-view reader-text-block__heading-3\">Key Points<\/h3>\n<ol>\n<li><strong>Challenges of Awake Prone Positioning in Non-COVID ARDS:<\/strong> While prone positioning is a well-established intervention in intubated ARDS patients, its benefits in non-intubated cases vary due to discomfort, positional intolerance, and inconsistent lung recruitment.<\/li>\n<li><strong>Use of Electrical Impedance Tomography (EIT):<\/strong> EIT is a non-invasive, radiation-free bedside tool that provides real-time visualization of lung ventilation, enabling the identification of optimal positioning strategies.<\/li>\n<li><strong>Case Presentation and EIT-Guided Positioning:<\/strong> A 61-year-old female with chemotherapy-induced lung toxicity developed Pneumocystis jirovecii pneumonia (PCP)-associated ARDS. Standard APP was poorly tolerated, prompting the use of EIT to guide alternative positioning.<\/li>\n<li><strong>&#8220;Thinker\u2019s Position&#8221; as an Alternative to Standard Proning:<\/strong> EIT analysis identified the &#8220;Thinker\u2019s Position&#8221; as the most effective for improving oxygenation (SpO\u2082\/FiO\u2082 ratio) and respiratory effort while minimizing discomfort.<\/li>\n<li><strong>Objective Monitoring of Oxygenation and Ventilation Indices:<\/strong> EIT showed that both the prone position and &#8220;Thinker\u2019s Position&#8221; improved end-expiratory lung volume (EELV), but the latter was better tolerated, allowing for prolonged use.<\/li>\n<li><strong>Comparing Different Positions:<\/strong> The global inhomogeneity (GI) index, a marker of uneven lung ventilation, was lowest in the prone position, followed by the &#8220;Thinker\u2019s Position.&#8221; However, the patient\u2019s ability to sustain the prone position was limited.<\/li>\n<li><strong>Successful Weaning from HFNC:<\/strong> The patient maintained the &#8220;Thinker\u2019s Position&#8221; for approximately 6 hours per day, leading to progressive respiratory improvement and successful weaning from HFNC after 12 days.<\/li>\n<li><strong>Impact on Respiratory Rate and ROX Index:<\/strong> Despite initial variability, the patient\u2019s respiratory rate gradually stabilized, and the ROX index\u2014an indicator of HFNC success\u2014improved over time, reinforcing the effectiveness of EIT-guided positioning.<\/li>\n<li><strong>Avoiding Discomfort-Induced Hypoxia:<\/strong> The patient\u2019s attempt to return to standard proning on day 3 resulted in significant discomfort and worsening hypoxia, further supporting the need for individualized positioning approaches.<\/li>\n<li><strong>Future Implications for ARDS Management:<\/strong> The findings highlight the potential of EIT as a precision tool for guiding APP strategies in non-intubated ARDS patients, reducing unnecessary discomfort while optimizing lung recruitment.<\/li>\n<\/ol>\n<p><a href=\"https:\/\/ccforum.biomedcentral.com\/articles\/10.1186\/s13054-025-05332-8\"><strong>ACCESS FULL ARTICLE HERE<\/strong><\/a><\/p>\n<p><b>Open Access<\/b>\u00a0This article is licensed under a Creative Commons Attribution 4.0 International License, which permits use, sharing, adaptation, distribution and reproduction in any medium or format, as long as you give appropriate credit to the original author(s) and the source, provide a link to the Creative Commons licence, and indicate if changes were made.
