However, there is no standard method to set and adjust the APRV mode shown to be optimal. Therefore, we performed a meta-regression analysis to evaluate how the four individual APRV settings impacted the outcome in these patients.<\/div>\n<div class=\"html-p\"><b>Methods:<\/b>\u00a0Studies investigating the use of the APRV mode for ARDS patients were searched from electronic databases. We tested individual settings, including (1) high airway pressure (P<sub>High<\/sub>); (2) low airway pressure (P<sub>Low<\/sub>); (3) time at high airway pressure (T<sub>High<\/sub>); and (4) time at low pressure (T<sub>Low<\/sub>) for association with PaO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0ratio and ICU length of stay.<\/div>\n<div class=\"html-p\"><b>Results:<\/b>\u00a0There was no significant difference in PaO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0ratio between the groups in any of the four settings (P<sub>High<\/sub>\u00a0difference \u221212.0 [95% CI \u2212100.4, 86.4]; P<sub>Low<\/sub>\u00a0difference 54.3 [95% CI \u221252.6, 161.1]; T<sub>Low<\/sub>\u00a0difference \u221227.19 [95% CI \u2212127.0, 72.6]; T<sub>High<\/sub>\u00a0difference \u221251.4 [95% CI \u2212170.3, 67.5]). There was high heterogeneity across all parameters (P<sub>hHgh<\/sub>\u00a0I<sup>2<\/sup>\u00a0= 99.46%, P<sub>Low<\/sub>\u00a0I<sup>2<\/sup>\u00a0= 99.16%, T<sub>Low<\/sub>\u00a0I<sup>2<\/sup>\u00a0= 99.31%, T<sub>High<\/sub>\u00a0I<sup>2<\/sup>\u00a0= 99.29%).<\/div>\n<div class=\"html-p\"><b>Conclusions:<\/b>\u00a0None of the four individual APRV settings independently were associated with differences in outcome. A holistic approach, analyzing all settings in combination, may improve APRV efficacy since it is known that small differences in ventilator settings can significantly alter mortality. Future clinical trials should set and adjust APRV based on the best current scientific evidence available.<\/div>\n<div>\n<h3><\/h3>\n<h3 id=\"ember64\" class=\"ember-view reader-text-block__heading-3\">Key Points<\/h3>\n<ol>\n<li><strong>Variability in APRV Application:<\/strong> The study highlights significant heterogeneity in how APRV is applied across clinical trials, with inconsistent settings for pressure levels and inspiratory\/expiratory times.<\/li>\n<li><strong>No Independent Effect of Individual Settings:<\/strong> Meta-regression analysis found that variations in PHigh, PLow, THigh, and TLow were not independently associated with significant differences in PaO\u2082\/FiO\u2082 ratio or ICU length of stay.<\/li>\n<li><strong>High Heterogeneity Among Studies:<\/strong> The included studies exhibited substantial differences in APRV protocols, contributing to inconsistent results and limiting generalizability.<\/li>\n<li><strong>Potential for Synergistic Effects:<\/strong> The authors suggest that APRV may be most effective when all settings are optimized together rather than focusing on a single parameter.<\/li>\n<li><strong>Role of TLow in Lung Protection:<\/strong> TLow, which regulates expiratory time, is critical for maintaining alveolar stability.
