This study aims to evaluate the clinical and theoretical limitations of the SpO<sub>2<\/sub>\/FiO<sub>2<\/sub>-ratio when using it to classify patients with ARDS and to follow disease progression.<\/p>\n<h3 class=\"c-article__sub-heading\" data-test=\"abstract-sub-heading\">Methods<\/h3>\n<p>Observational cohort study of ARDS patients from three high-resolution Intensive Care Unit databases, including our own database ICU Cockpit, MIMIC-IV (Version 3.0) and SICdb (Version 1.0.6). Patients with ARDS were identified based on the Berlin criteria or ICD 9\/10-codes. Time-matched datapoints of SpO<sub>2<\/sub>, FiO<sub>2<\/sub>\u00a0and partial pressure of oxygen in arterial blood (PaO<sub>2<\/sub>) were created. Severity classification followed the thresholds for SpO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0and PaO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0of the newly proposed global definition.<\/p>\n<h3 class=\"c-article__sub-heading\" data-test=\"abstract-sub-heading\">Results<\/h3>\n<p>Overall, 708 ARDS patients were included in the analysis. ARDS severity was misclassified by SpO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0in 33% of datapoints, out of which 84% were classified as more severe. This can be partially explained by imprecision of SpO<sub>2<\/sub>\u00a0measurement and equation used to transform SpO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0to PaO<sub>2<\/sub>\/FiO<sub>2.<\/sub>\u00a0A high dependence of SpO<sub>2<\/sub>\/FiO<sub>2<\/sub>-ratio on FiO<sub>2<\/sub>\u00a0settings was found, leading to major treatment effect and limited capability for tracking change in ARDS severity, which was achieved in less than 20% of events.<\/p>\n<h3 class=\"c-article__sub-heading\" data-test=\"abstract-sub-heading\">Conclusions<\/h3>\n<p>The use of SpO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0interchangeably with PaO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0for severity classification and monitoring of ARDS is limited by its inadequate trending ability and high dependence on FiO<sub>2<\/sub>\u00a0settings, which may influence treatment decisions and patient selection in clinical trials.<\/p>\n<h3 id=\"ember66\" class=\"ember-view reader-text-block__heading-3\">Key Points<\/h3>\n<ol>\n<li><strong>Misclassification of ARDS Severity by SpO\u2082\/FiO\u2082:<\/strong> The study found that SpO\u2082\/FiO\u2082 misclassified ARDS severity in 33% of cases, with 84% of misclassifications resulting in overestimation of severity compared to the PaO\u2082\/FiO\u2082 ratio.<\/li>\n<li><strong>Dependence on FiO\u2082 Settings:<\/strong> The SpO\u2082\/FiO\u2082 ratio was highly dependent on FiO\u2082 settings, leading to inaccurate classification. The study observed the lowest accuracy (22%) when FiO\u2082 was set at 70%, suggesting that treatment-related FiO\u2082 changes could distort severity assessment.<\/li>\n<li><strong>Poor Trending Ability for Disease Progression:<\/strong> The SpO\u2082\/FiO\u2082 ratio failed to reliably track changes in ARDS severity over time, correctly identifying severity progression in only 19.6% of cases. This limitation affects its utility for monitoring ARDS patients.<\/li>\n<li><strong>Comparison with PaO\u2082\/FiO\u2082 Ratio:<\/strong> While SpO\u2082\/FiO\u2082 correlated moderately with PaO\u2082\/FiO\u2082 (r = 0.69), it demonstrated significant variability, making it unreliable for direct substitution.<\/li>\n<li><strong>Influence of Oxygen Saturation Variability:<\/strong> SpO\u2082 values were affected by measurement biases, including sensor variability and perfusion status. The study found a mean SpO\u2082 overestimation bias of +0.5% compared to SaO\u2082, with greater discrepancies at higher oxygen saturations.<\/li>\n<li><strong>Limitations in Conversion Equations:<\/strong> Various conversion formulas used to estimate PaO\u2082\/FiO\u2082 from SpO\u2082\/FiO\u2082 performed inconsistently.
