Despite 60 years of research, mortality remains near 40%. The heterogeneity of ARDS has driven interest in subphenotypes\u2014biological, clinical, physiological, and radiographic\u2014that may explain variable responses to therapies. This review summarizes current pathophysiological insights, describes key subphenotypes, and highlights implications for personalized therapeutic strategies.<\/p>\n<p><img fetchpriority=\"high\" decoding=\"async\" class=\"aligncenter wp-image-1415 size-full\" src=\"https:\/\/perfusfind.com\/ic\/wp-content\/uploads\/2026\/03\/1757119880729.png\" alt=\"\" width=\"1373\" height=\"1000\" srcset=\"https:\/\/perfusfind.com\/ic\/wp-content\/uploads\/2026\/03\/1757119880729.png 1373w, https:\/\/perfusfind.com\/ic\/wp-content\/uploads\/2026\/03\/1757119880729-300x218.png 300w, https:\/\/perfusfind.com\/ic\/wp-content\/uploads\/2026\/03\/1757119880729-1024x746.png 1024w, https:\/\/perfusfind.com\/ic\/wp-content\/uploads\/2026\/03\/1757119880729-768x559.png 768w\" sizes=\"(max-width: 1373px) 100vw, 1373px\" \/><\/p>\n<blockquote id=\"ember66\" class=\"ember-view reader-text-block__blockquote\"><p><strong>Key Insights:<\/strong><\/p><\/blockquote>\n<ol>\n<li>Barrier dysfunction occurs when epithelial and endothelial injury increase permeability, leading to protein-rich edema, impaired clearance, and neutrophil\/platelet-driven inflammation.<\/li>\n<li>Inflammation is driven by neutrophils, macrophages, and cytokines such as IL-1\u03b2, IL-6, IL-8, and TNF-\u03b1, which propagate both local damage and systemic MODS, underscoring that ARDS is more than a lung disease.<\/li>\n<li>Mechanical forces from ventilation cause volutrauma, barotrauma, atelectrauma, and biotrauma, amplifying injury. Driving pressure is the strongest ventilatory predictor of mortality.<\/li>\n<li>Biological subphenotypes include the hyperinflammatory group, marked by high cytokines and vasopressor use with worse outcomes, and the hypoinflammatory group, with lower inflammation and better survival; they show distinct responses to fluids, PEEP, and statins.<\/li>\n<li>Clinical subphenotypes demonstrate that trauma-related and non-sepsis ARDS patients have better outcomes compared with sepsis-driven ARDS, while late-onset ARDS is linked to higher mortality.<\/li>\n<li>Physiological subphenotypes are defined by PaO\u2082\/FiO\u2082 ratio, compliance, dead-space fraction, and recruitability, with low compliance and high dead-space fraction predicting mortality.<\/li>\n<li>Radiographic subphenotypes distinguish focal ARDS, characterized by dorsal opacities and strong responsiveness to prone positioning, from non-focal ARDS, which presents diffuse opacities and is recruitable with PEEP. Subphenotypes influence outcomes and therapeutic responses, and their accurate identification may finally unlock precision therapies.
